We want to make your experience with us as hassle-free and pleasant as possible. The more you prepare for the visit, the more relaxed you will be on the day, and the less time you will need to take from your busy schedule. Here are a few things you can do beforehand to help that along.
Set Up Your Appointment
You can use our Appointment Request Form to schedule your first visit online. You will be asked to choose an office location convenient for you. Fill Out the New Patient Form
You can save yourself time at your first visit by downloading, printing, and filling out our New Patient Registration Form before you come. If you need Adobe Reader to access the form you can find it here.
This form includes a place to list your current medications and supplements as well as pertinent medical history. Please fill out as much of the form as you can. Our staff is always happy to assist new patients with any questions on the day of your visit.
Items to Bring with You
- Your insurance card (we need a copy for our records)
- A form of photo identification (many insurance companies will not pay benefits without it)
- A referral from your primary care physician, if your plan requires it (contact them for that document and bring it with you, or your visit may not be covered by insurance)
- Your filled-out patient registration form
- A means of paying any deductible or copay that you may owe at the time of service (we accept cash, checks, debit cards, and most major credit cards, as well as PayPal and prepaid gift cards)
- A list of your questions for our foot specialists
Other Helpful Things to Take Along
A pair or two of your shoes—work, athletic, and casual or dress shoes—can be very helpful. Examining the wear on them can help us determine what is causing your foot issues.
The same holds true for any over-the-counter shoe inserts or custom orthotics you have been using. Sometimes these were not designed properly in the beginning, or they wear out, or your foot changes so they do not work as they were intended. We can find out if they are still beneficial to your feet or causing them harm.
